Start with a visit
Appointments are booked in Jane. Before the first session you will receive an intake form, a cost estimate, a PHIPA consent form, and the cancellation policy, plus notes on what to bring.
Please do not put a detailed medical history in the form below. Intake is the safer place for that.
Before you come
- Complete the intake form so we can estimate the assessment fee.
- Send any swallow study reports ahead of a feeding visit.
- Full fees apply with less than 24 hours’ notice.
